Usé npx create-react-app@latest my-app. esto funcionó para mí

intente borrar npx-cache si recientemente ejecutó esto en una versión anterior.
npx clear-npx-cache npx install -g create-react-app react-app

También lo que se puede hacer es entrar a la consola como administrador, ubicado en la carpeta del usuario, y poner este comando (en mi caso usé el de NPM)
npm desinstalar -g crear-reaccionar-aplicación
lo desinstalamos y luego lo reinstalo
npm install -g crear-reaccionar-aplicación
y listo, me funciono

Así que estos son los pasos que hice para que funcione:
npm uninstall -g create-react-app npx create-react-app@latest my-app-name

borrar el caché npx resolvió el problema para mí:
npx clear-npx-cache
