how can this source path be fixed to allow for the html page to load the javascript script and background image

This is the java script link that I have in html

This is the image link I have in css

This is my file paths

everytime I load the submit.html webpage I get this error enter image description here

I know the error has something to do with the way my paths are set up but I cant pinpoint exactly whats wrong.
