¿Cómo refactorizo este código para que no arroje este error "La propiedad 'paciente' no existe en el tipo 'Solicitud <ParamsDictionary>"?

Aquí está mi código. Estoy tratando de usar la autenticación de pasaporte para almacenar la información de los pacientes específica del token creado para cada persona.
