I want to create a Health Grantt Chart that can show a patient past medical history. During his lifetime, it can show if he/she has gone through any injuries or suffered from any diseases. For instance, if a patient is 5 years old, it'll show his immunization, surgeries, and some other health events in columns and the values over the years.

I'll need some ideas, from where to start, the right tools to be used for such a job, ...etc.
